The patient death register includes name, age, date admitted, birthdate, place of birth, religion, marital status, doctor, related family, drug store, funeral home, social security number, medicare name and insurance company. The death record includes name, where born, date admitted, death date, buried in grave marked (some include birth date, funeral home, cemetery, etc). The register includes name, age, sex, birth place, previous habits, present condition, present location, remark.
