Microfilm of original records at the Cook County Courthouse, Chicago, Illinois.

Birth correction certificates correct original birth certificates years after (even decades after) the completion of the original certificates.

All these records, located in the "Film Notes," are in loose alphabetical order.

Included are general indexes that divide those records that are indexed. Birth correction certificates are usually indexed by the original name not the corrected name.

The corrected names are usually indexed in “Cook Co., Ill. Birth Index, 1871-1916.”

The last groups of records in the “Film Notes” are NOT indexed.

Includes some unrelated, actual birth records, "Report of Birth" & "Certificates of Birth." These records have not been located in any of the library’s indexes & they date from the 1870s to the 1940s.
