SPECIAL INSTRUCTIONS FOR SEARCHING THIS RECORD: The Death Record volume number will usually be found only on the beginning page of the volume. The date of death is the key to locating the Death Record. Search the record microfilm to locate the volume number; the page number may be either written or stamped in the upper left hand corner. The death date is important because certificate numbers in some sections go up to 500 and then repeat; search before and after the assumed certificate number since the dates are not in sequence.

Early registration of vital records in Wisconsin was incomplete. Most of the deaths are from the 1850s.

The reel numbers, image numbers, and sequence numbers in this index refer to the films housed in the state of Wisconsin files. (not the Genealogical Library's films).

There are about 28 questions asked on each death certificate, such as: name of deceased, maiden name, color, sex, occupation, age, birthplace, name of father, father's birthplace, name of mother, mother's birthplace, name of spouse, birth date, death date, residence at death, place of burial, disease and duration, and place of death.
