- Family Group Record: Patrons may use this form to record information about a family group.
- Pedigree Chart: Patrons may use this form to record information about 4 generations of ancestors.
- Research Log: Patrons may use this form to record sources for a research goal.
- Research Log and Next Steps: It is a modified research log to help patrons track sources for a research goal, along with the next steps in their research.
- Permission to Duplicate: Guests can use this form to give permission to FamilySearch to duplicate donated family history materials.
