Important: The microfiche associated with this title are restricted, meaning they can only be obtained in the Family History Library at the Access Services Window on the 2nd floor.

This index is used for finding the following record types held by the library: (1) birth certificates; (2) birth entries in birth registers. All library titles indexed by this record are found in the "References" field below. This index is arranged in alphabetical order under the following column headings: "Last Name," "First Name" and middle initial--"INT." Unnamed infants, however, are symbolized by an ampersand (&) in the INT column. The names included before an ampersand, in the "First Name" column, are the father's first initial and the mother's first given name.

Other column headings include: "Birth Date" (month, day & year), "Birth Place", and "ID#." The birth place will be either "Chicago" or "Cook County." The ID# (identification number) may be a certificate number or a reference to a birth register or both. When an ID# begins with a letter (A-E) it is referencing a register only, not a birth certificate. The numbers following the letter are page & entry numbers.

Other symbols found in identification numbers include DS & ODS. DS refers to a Delayed Series of certificates. The library does not have these certificates. ODS refers to an Old Delayed Series of certificates. The library also lacks these delayed certificates. Contact the Cook County Clerk's Office for copies of delayed certificates.
